New Patients

Dr. Vincenzo and staff are committed to providing you with a pleasant visit. Our administrative staff is ready to help you with question about scheduling, financial policy and insurance to make that part of the process as simple as possible.

You may download and print the new patient forms. We ask that you fill them out and sign them, bringing them with you to your first visit. Also a list of the medications you are currently taking as well as any insurance information should be brought along.
